Posting Summary

Rutgers, The State University of New Jersey is seeking a Health Care Case Manager for the Francois-Xavier Bagnoud Center – Child Health Unit of School of Nursing. The Health Care Case Manager functions to support the Division of Youth and Family Services (DYFS)/Department of Children and Families (DCF) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with the other staff of the Child Health Unit in that local office. The Health Care Case Manager provides nursing case management services in home, local office and health care settings. Nursing case management services include nursing assessment of a child’s health status and care needs, development of a health plan, maintenance of a medical passport/health summary, ongoing monitoring, coordination of care, advocacy, and health education to assure that each child in out-of-home placement receives appropriate health care, per EPSDT and AAP guidelines.

  • Assists with enrollment of child in managed care entity and identification of a medical home upon entry into out-of-home placement; informs the child, caregiver, and DCF/CP&P.;
  • Assesses child’s health care status and needs by retrieving a complete health history through obtaining and reviewing all medical records and interviewing past providers and caregivers; forwards all information to the medical home and other health providers.
  • Schedules an Initial Physical Health Evaluation, in consultation with caregivers and DCF/CP&P; reviews the preliminary and final reports, and ensures that all recommendations for health care are completed.
  • Develops a Child Health Care Plan in collaboration with the child’s medical home, specialists, caregivers, and other providers.
  • Ensures that the child is scheduled for and attends timely EPSDT visits and specialty appointments; reviews all reports and forwards necessary reports to the medical home.
  • Provides ongoing monitoring of children’s health, behavioral health, developmental and educational status, and ensures that appropriate health care and services are in place.
  • Maintains systematic communication, contact, and consultation with the child’s caregivers and the DCF/CP&P; case management team; educates caregivers and DCF/CP&P; staff about health issues,which are pertinent to child’s care.
  • Facilitates transfer of care and records to an identified medical home and HMO care manager when the child is changing placement or upon reunification or permanent placement.
  • Provides assistance when accidents or illnesses occur in workplace emergencies; performs CPR as indicated and notifies 911 and supervisor.
  • Participates in continuous quality improvement activities and research data collection.

Minimum Education and Experience
  • Bachelor’s Degree in Nursing or a BA/BS in a health-related field; may substitute years of experience.
  • Current license as a Registered Nurse in the State of New Jersey.
  • A minimum of 1-2 years of pediatric nursing experience.
Required Knowledge, Skills, and Abilities
  • Current certification in American Heart Association Basic Life Support for Health Care Providers.
  • Computer literacy/computer skills.
Preferred Qualifications
  • Two (2) years of experience in nursing case management, community health nursing, or nursing care of vulnerable families.
Physical Demands and Work Environment
  • Requires the ability to lift up to 50 pounds (infants/children) to waist level.
